You can get our easy application and certification in two ways. Click on your choice below:


If you like, you can complete the application and certification and return it by mail or by email: AmplifyLife@regstaff.sc.gov

Printed on the back of the application is a brief certification form. Just take it with you when you meet with a professional certifier, who can be any one of the following:

  • Audiologist
  • Physician
  • Physician’s Assistant
  • Advanced Practice Registered Nurse
  • Speech-Language Pathologist
  • Hearing Aid Specialist


The certifier will sign the form to confirm that your permanent disability makes it difficult to use the phone.

There is a wide selection of products to choose from. And we make it a breeze to find the product that fits your needs.

Please DO NOT send original documents. We only need photocopies of these very important documents to verify your identity and South Carolina residency:

  • Copy of a valid SC driver’s license, SC identification card, or SC voter 
        registration.
  • Copy of a page from your current phone bill showing your phone 
        number and address.

Simply put these items into an envelope and mail it back to us:

  • Application Form completed with professional certifier signature
  • Copy of a valid SC driver’s license, SC identification card, or SC voter 
        registration.
  • Copy of a page from your current phone bill showing your phone 
        number and address.
